Copays and Deductibles
Payments are collected automatically after your session. Please be sure to contact your insurance to find out your copay or the remainder of your deductible. We are also able to bill through HSA cards.

Here is a downloadable superbill pdf for you to review. The document would be completed by your provider and given to you to submit to your insurance for reimbursable out-of-pocket expenses.
